I apply for the Foundation agility course at Akarana Dog Training Club and agree to abide by and be bound by the rules and regulations of this club while participating in club activities.

I agree that my dog will have vaccinations kept up to date, and I will inform the instructor(s) if my dog has nervous tendencies and/or mouthing issues that may be interpreted as aggression and I will be responsible for all costs incurred as a result of my dog's actions. I will notify Akarana DTC if my dog has a menacing/dangerous dog order (or similar issued by any local authority) against them at any time during the course of my enrolment.
